Description:

Clinical Focus: This set of values is intended to capture that a patient is ordered, admitted to, or referred to palliative care, comfort measures, or Hospice care.

Data Element Scope: Appropriate for use with data elements in QDM Categories or Attributes represented by procedure codes.

Inclusion Criteria: This includes a referral or admission for palliative care or the health care provider is seeing this patient for palliative care.

Exclusion Criteria: Excludes codes that do not meet the inclusion criteria.
